If you are not a CAN patient, please ensure to reach out to establish care no later than April 15th to ensure a smooth transition. Enrolling as soon as possible helps ensure your care continues without interruption.

 

What This Means for You

If your insurance premium assistance is ending:

  • You could lose your current health insurance coverage
  • From March 1st to April 30th, you can elect changes to your ACA plan, or be evaluated for CANCares Premium Assistance Plan for eligibility.
  • Taking action quickly can help prevent gaps in care or medication access
